Hear what others have to say about working with VPCN

The collaboration between my physician’s office and the PCN social worker has been invaluable in the maintenance of my health. I've been supported and connected to resources in the community, which have been stabilizing factors in my life during a health crisis. I don’t know where I’d be right now without this collaboration, and I’m so grateful for them and this service.
