Patient Medical Homeal Pharmacists provide comprehensive medication management services focused on the ongoing care of adult patients with complex conditions to prevent and resolve medication-related problems. They also provide education to patients about their medications and address barriers to adherence. PCCPs collaborate with the patient and healthcare team to implement treatment plans. Role
- Uses their special training to ensure patient medications are safe and effective
- Will work in a Patient Medical Home (PMH), in another healthcare setting, or virtually
- Will not dispense medications; willl work together with the pharmacist and pharmacy where prescriptions are filled
- Will meet with patients to help fully understand health needs and answer any questions
- Will treat patients with dignity and respect and ensure patients receive care in a way that feels safe to them
Scope of Care
Can see complex patients for:
- Multiple drug therapies or health conditions
- Optimizing drug therapy for chronic diseases
- Polypharmacy concerns
- Medication allergies or adverse reactions
- Suboptimal drug therapy outcomes
- Complexities from self-treatment including supplements
- Any medical complexity, issues, or concerns
Can see patients experiencing change due to:
- Starting or stopping medications
- Recent discharge from hospital
- Recent attachment to a new provider
- Complex or costly drug therapies
Qualifications
- Licensed by, or eligible for licensing by the College of Pharmacists of British Columbia
- Completion of an Accredited Pharmacy Residency or an equivalent combination of education, training and experience
