Mental Health Substance Use Health Consultant

The Primary Care Network MHSU Health Consultant service is a quick response program for Family Physicians and Nurse Practitioners to connect their patients with a dedicated Master’s-prepared health clinician. If you’re interested in helping patients who present with mild to moderate mental health and substance use issues who could benefit from short-term counselling and/or system navigation, this role with the Victoria PCN could be the one for you!
Role
Service to FPs/NPs
As part of a dedicated team of MHSU Health Consultants exclusive to Victoria PCN members, you would provide:
- Patient risk assessment
- Case conferencing
- Linkage to crisis outreach
- System navigation
- Team-based care planning
Service to Patients
This role supports timely access to solution-focused therapy for mild and moderate MHSU issues and may include:
- Mental health assessment and substance use assessment/screening
- Care planning and system navigation
- Short-term solution-focused counselling
- Bridging counselling support while waiting for higher level of care
- Coaching with self-management of health issues
Scope of Care
- Works collaboratively to deliver team-based care as a member of a multidisciplinary care team that includes Family Physicians, Nurse Practitioners, MOAs, Indigenous Wellness Providers, Clinical Pharmacists, and Registered Nurses
- Supports a small cohort of Family Physicians and Nurse Practitioners and their attached patients
- Provides advanced clinical practice and interventions in the community setting
- Provides services that are continuous in all phases of mental health, substance use, and concurrent disorder care and support
- Carries out quality assurance activities and projects, analyzing and acting upon information received through incident reports, evaluations, and other data, including collecting and compiling statistical data for research and evaluation purposes and contributing to the development of outcome measures for Mental Health Services
- Ensures a safe and healthy working environment by observing universal precautions and infection control procedures; removing obvious hazards; reporting faulty equipment, accidents, injuries and near misses; and adhering to and enforcing rules regarding safety
- Communicates and shares expertise and information with other health care professionals, community agencies, and the public about mental illness and mental health services, promoting best practices, good public relations, and facilitating an efficient and cooperative relationship
- Performs other related duties as required
Qualifications
Master’s degree in a health-related discipline from an approved post-secondary institution. Recognized post basic education related to mental illness, substance use and concurrent disorders; two (2) years recent, related experience; a significant understanding of mental health issues; substance use issues and concurrent disorders issues and evidence based best treatment practices; knowledge of applicable legislation, such as the Mental Health Act and the Guardianship Act; knowledge of available programs, their interrelationships, and their function in delivering care in the community to persons with mental illnesses, substance use and/or concurrent disorders; knowledge of mental health/substance use assessments, crisis intervention, treatment methodologies, intervention strategies, the change process, and community dynamics; or an equivalent combination of education, training and experience. Valid BC driver’s license.
